A useful list of links for your directory, for Italy, is the online Dictionary of Italian Librarians (based on the website of the Italian Library Association) http://www.aib.it/aib/editoria/dbbi20/dbbi20.htm
The most-known poets-librarians are Montale, Guerrini, Di Giacomo (already listed) and Domenico Gnoli (not in your list, also known as Giulio Orsini). Other well-known poets-librarians from Italy are Manlio Dazzi, Renato Fucini, Giuseppe Guglielmi, Biagia Masulli-Marniti, Ubaldo Mazzini, Ettore Moschino, Giggi Zanazzo. But a lot of others wrote poems, while they are not widely known neither as poets nor as librarians.
Among living persons, the most famous is perhaps Francesco Leonetti (http://www.treccani.it/enciclopedia/francesco-leonetti/ in the most important Italian encyclopaedia, but it does not mention his work at the Archiginnasio Library in Bologna and as director of the Malatestiana Library in Cesena).
